Guide for Filing Federal Civil Suits Against the United States, an Agency Official, or an Employee of the United States Government in the Northern District of Texas

If you want a file-stamped copy for your records, you must provide one copy in addition to the numbers stated below.

ITEM REQUIREMENTS
Civil Cover Sheet (JS-44) Submit the original and one copy to the Clerk’s Office.
Complaint Submit the original and one copy to the Clerk’s Office.

Submit one copy in person or via registered or certified mail to the United States Attorney’s Office (addressed to Civil Process Clerk) at one of the following addresses:

Submit one copy to the Attorney General of the United States via registered or certified mail at the following address:

950 Pennsylvania Avenue, NW, Washington DC 20530-0001

Submit one copy to each agency, officer, or corporation listed on the complaint via registered or certified mail.

Certificate of Interested Persons File the original plus one copy for the Court.  See LR 3.1(f).
Summons Submit three summonses each for service on the following: 1) the U.S. Attorney; 2) the Attorney General; and 3) each agency, officer or corporation listed on the complaint. One summons is for service with the complaint, one is for return of service, and one is for the Clerk’s Office file.

Pursuant to Rule 12 of the Federal Rules of Civil Procedure, the United States or any agency or official of the United States shall have 60 (not the usual 20) days to answer the complaint. Enter "60" in the applicable place on each summons.

Court Fees The filing fee is $350.00 for each case. Make checks payable to "Clerk, U.S. District Court."
